È uno dei capolavori cosmateschi di Jacopo e Pietro Vassalletto. Lo spazio quadrangolare è scandito da colonne lisce, tortili e intrecciate, arricchite da mosaici e peculiari elementi ornamentali. Notevoli i capitelli decorati a motivi zoomorfi, antropomorfi e vegetali, il fregio perimetrale e le statue di sfingi e leoni. Da ammirare gli antichi battenti in bronzo della Scala Santa e nel terzo ambulacro la tomba di Riccardo Annibaldi, prima opera romana di Arnolfo di Cambio (1276).

The masterpiece of the two famous roman architects whose cosmatesque style was very fashionable during the Middle Ages. The small columns of the lateran cloister alternate between simple and braided, enriched by colored weaves, like a mosaic, and sculpted capitals. Noteworthy are the friezes inside and the human and lion masks on the exterior. Among the masterpieces are the ancient Battenti in bronze of the Scala Sancta. In the third ambulatory is the first roman work by Arnolfo di Cambio, the Tomb of Riccardo Annibaldi (1276).
